RAI Manual v1.13 is available in the Downloads section on the CMS sire. MDS 3.0 RAI Manual v1.13 and Change Tables_October 2015.zip contains the full set of chapters, sections, appendices, and change tables.  MDS 3.0 RAI Manual v1.13 Replacement Manual Pages and Change Tables_October 2015.zip contains only the pages that changed from v1.12R to v1.13 and the changes table corresponding chapters, sections, and appendices.

SOME IMPORTANT CHANGES: Please note that, unlike prvious versions, version 1.13 has a footer date of October 2015 on every page, and the Table of Contents no longer lists different version numbers for each chapter/section.  Second, the change tables no longer list corrections to minor formatting, punctuation, or typhographical errors that do not affect interpretation of content.  Numerous formatting corrections have been made to ensure that RAI Manual v1/13 meets the accessibility requirements of Section 508 of the Rehabilitation Act, especially in Appendix C and Chapter 3 pages with "definition boxes."  These pages may look different from previous versions, but replacement pages have not been provided because the changes did not affect content.



October 2014:   Updates and changes to the RAI manual effective 10/1/14.
          This version of the MDS 3.0 RAI manual incorporates clarifications to existing coding and transmission policy, integrates previously               published Quaestions and Answers (Q & A's) into the appropriate sections and addresses requested clarifications and scenarios                 concerning complex areas.

At the June 13 Skilled Nursing Facility/Long-term Care open Door Forum, officails with the Centers for Medicare and Medicaid Services (CMS) announced that there is an error on page 5-2 of the RAI User's Manual for the MDS 3.0 that occurred with the implementation of the May 2013 manual updat (version 1.10).  Section 5.2 on page 5-2 details the timeliness criteria for MDS assessments.  Under ompletion Timing," the second bullet currently states:
For the Admission assessment, the MDS Completion Date (Z0500B) must be no later than 13 days after the Assessment Reference Date (ARD) (A2300).
This statement is "erroneous," stressed officials. The bullet should read as follows:
For the Admission assessment, the MDS Completion Date (Z0500B) must be no later than 13 days after the entry date (A1600).  
"The admission assessment timing is accurately reflected in Chapter 2 of the manual,"  stated officials.  
The Chapter 5 error will be corrected in the next update.

May 2013:  Updates to the manual have been published by CMS.

November 2012: The Centers for Medicare & Medicaid Services (CMS) recently posted MDS 3.0 Resident Assessment Instrument (RAI)
Manual v1.09 Replacement Change Table and Manual Pages.

New F-tags for tube feedings were also released.  Your administrators should have them or your dietitian. 

October 2012:  CMS has released version 6.0 of the 
QM User's Manual as well as a Quality Measure Identification Number by CMS
Reporting Module table which documents CMS quality measures calculated using MDS 3.0 data and reported in a CMS reporting module.

The following Modules are included:
     *  CASPER Reporting Quality Measure Reports
     *  Nursing Home Compare (NHC)
     *  Five Star Quality Rating System
     *  Facility and Resident Quality Measure Preview reports

October 1, 2012:  THERAPY CAP EXCEPTION.  
CMS will implement a phased Therapy Cap Exception process. Check with your therapy providers.  They should have the current information. the official instruction, CR8036, issued to your carrier, FI, or A/B MAC regarding this change may be viewed at http;//www.cms.hhs.gov/Regulations-and-Guidance/Guidance/Transmittals/Downloads/R1117OTN.pdf on the CMS website

JFS will reimburse Medicaid residents differently when the RUG score is a PA1 or PA2.  Residents will be billed with a different revenue code. A flat fee of $130 will be in place for these scores.

April 1, 2012:  New updates to the RAI manual have been posted on the CMS website.
Quality measures are back and can be printed from the CMS website.
